Open a finished FLHA. Look inside an audit package.

Two documents the platform actually produces, from a fictional sample company. No login, no form to fill in.

These are sample documents produced by the software — not a customer's records. The company, jobs, sites and workers in them are invented, and every page is stamped “SAMPLE RECORD — not a real safety record”. No customer's documents, names or records are used anywhere on this page.

The paperwork, before you talk to anyone

Every form submitted in the portal generates a PDF. Those PDFs are what an auditor or a prime contractor asks for — here is the shape of them.

A finished FLHA, as a PDF

One Letter page, generated when a crew submits: company header band, a detail grid for project, site, prime contractor and task, the hazards with their controls and PPE, crew sign-off, the emergency plan and the supervisor authorization line.

A sample COR audit package

A zip with the folder-per-form-type structure the export produces, the same file naming, and a README inside explaining what a full export adds. Thirteen sample documents across five folders.

What an auditor opens

Records are filed by form type, with the record's own date in the file name — so a package for a three-month window reads like a binder with tabs.

  • Field Level Hazard Assessments/FLHA_YYYY-MM-DD.pdf3 documents
  • Toolbox Meetings/TOOLBOX_YYYY-MM-DD.pdf3 documents
  • Site Inspections/SITE-INSPECTION_YYYY-MM-DD.pdf3 documents
  • Worker Orientations/ORIENTATION_YYYY-MM-DD.pdf2 documents
  • Incident Records/INCIDENT_YYYY-MM-DD.pdf2 documents
  • README.txtWhat the package is, and what a full export addsroot of the zip

What the full export adds when you run it

  • A “Package built from N of M records” line. The confirmation tells you how many records in your date range went into the package, out of how many there were — so a gap is visible before the auditor finds it.
  • MISSING_FILES.txt when something can't be attached. If a record in range has no file link, or its stored file no longer loads, the zip still builds and that file names each record, its form type, its date and the reason.
  • Records outside the COR form types are counted and named on screen instead of disappearing from the package silently.
  • A Worker Certifications folder when certification photos are attached to the worker records.
  • Every record in the range you pick — the sample here is thirteen records from an invented company, not a full quarter's worth.

The five form types above are the ones in this sample. The platform's form library covers more — see the Forms page for the full list.
